The Massachusetts Care Coordination Network (MCCN) is here to provide MassHealth patients help with navigating the health care system, coordinating care plans and offering social supports to achieve the personal wellness goals of eligible individuals.
If you or someone you care for is a member of MassHealth, enrolled in an ACO or MCO organization and has life-long complex needs, then you may be eligible to receive services from MCCN. You and your family don’t have to manage chronic illness or disabling medical conditions on your own. Our team can evaluate your support systems and help to optimize your current care.
Contact us today to determine your eligibility. Our team can guide you through the enrollment process. If you are more comfortable, your health care provider can do this for you. Ask your provider to make a referral for MCCN services or bring this Request for Referral to your next doctors appointment.
